ID CARE SERVICES
How we tailor support
1. Personalised plans
co-written with families and professionals (EHCP/OT/SLT where relevant), specifying preferences, cultural and religious needs, sensory sensitivities, and communication styles.
2.
Voice and choice
We seek consent at every step (using visuals, social stories, or easy-read where helpful) and offer choices to build autonomy.
3. ndependence goals
step-by-step prompting (e.g., backward chaining) so skills grow safely over time; regular goal reviews with measurable outcomes.
4.
Right staff, right time
consistent key workers, 1:1 or 2:1 ratios as risk-assessed; gender-specific support on request.
Supporting Progress, Safety, and Independence Together

- Safe moving & handling; environment checks before any transfer.
- Clear escalation routes for health concerns, skin changes, or unexplained marks (body maps and incident reports where appropriate).
